Abstract


ObjectiveThis study focused on the changes of the expression level of Hexokinasell (HK I) during the transition from the chronic phase to blastic phase in the patients with chronic myeloid leukemia (CML), thus provided new clues for exposing the mechanisms of blastic crisis of CML and also provided a new way of diagnosis for monitoring the blastic crisis of CML so as to establish theoretical basis for developing new therapeutic target.MethodsThis study divided 41 selected cases into the experimental group (31 cases) with 14 cases with CML in the blastic phase and 17 cases with CML in the chronic phase and the control group (10 cases). The bone marrow fluid extracted from the two groups were measured by HK I.①At the transcriptional level, images were achieved by semi-quantitative PCR and then systematically analyzed to measure the value of Integrated Optical Density(IOD) by the image analysis software (Gel-Pro Analyzer V4.0) produced by Media Cybernetics in America. Finally the statistical analysis was conducted.②At the protein expression level, images were achieved by immunohistochemistry and then systematically analyzed to measure the value of Integrated Optical Density(lOD) by the image analysis software (Image-Pro-Plus V6.0) produced by Media Cybernetics in America. Then statistical analysis was conducted. The changes of the expression level of HK I in the CML patients with the chronic phase and the CML patients with blastic phase were explored.Results1. At the transcriptional level. the average value of IOD of the HK I PCR bands of 14 cases with CML in the blastic phase was 0.836±0.105 and the average value of IOD of 17 cases with CML in the chronic phase was 0.418±0.107 and in the control group with 10 cases, the average value of IOD was 0.122±0.051. It indicated the apparent increase of HK I expression level in the blastic phase than in the chronic phrase (P< 0.01) and that in the chronic phrase was higher than in the control group (P<0.01), so the HK I expression level decreased as follows:in the blastic phase, in the chronic phrase and then in the control group.2.·At the protein expression level, the IOD average·value of HK immunohistochemical staining of 14 cases with CML in the blastic phase was 1.230±0.187, the IOD average value of 17 cases with CML in the chronic phase was 0.671±0. 110while in the control group, the IOD average value was 0.214±0.083. It indicated the apparent increase of the IOD average value in the blastic phase than in the chronic phrase (P<0.01) and that in the chronic phrase was higher than in the control group (P<0.01). This indicated the expression level of HK I in the blastic phrase is higher than that in the chronic phrase.Conclusions1. The measured transcriptional expression level and the protein expression level both indicate that the HK I expression in the blastic phrase is higher than that in the chronic phrase and the HK I expression in the chronic phrase is also higher than that in the control group. The conclusion is consistent.2. The marrow HK I expression of the patients with chronic myeloid leukemia (CML) is higher than that of patients with non neoplastic diseases, which shows that the high expression of HK I is related to tumour and it is consistent with the the related research, and also shows that HK 1 participates in the pathogenesis of CML and thus can be made as the markers of the disease activity combined with other indexes.3. Compared with the HK I expression level of CML patients in the chronic phase the HK I expression level in the blastic phrase increases apparently and it is supposed that HK I is necessary to sustain the malignant phenotype of the cells of the patients with the blastic phrase and that the change of the HK I expression is another factor that impels the transition of CML from the chronic phrase to the blastic phrase.
